Uses: Quiz and Assess, Collect Feedback, Respond and Reflect

 

Description: Use this tool to easily create online forms complete with check boxes, multiple choice buttons, drop down menus, text boxes, etc. See a working example on my Contact Form.

 

Choose from a wide variety of response options, and then either embed the form in an existing web site (your PBWorks or a school site) or let the survey stand alone on its own webpage (a link is generated by the site). This guarantees that you receive an individual response from each person taking the survey. (For more details, check out Nick Peachey's summary of Jot Form).

 

If you're looking for a similar site that offers better options for compiling and analyzing the resulting data, but without a way to track exactly who is completing the forms, then check out QuestionPro instead.

 

 

 

Applications for the Reading/Language Arts Classroom:

 

  • Create "exit tickets" for the completion of any web task.
  • Collect parent feedback about student work on wikis (students can leave comments since they're members of the wikis, but parents cannot).
  • Allow students to create and embed their own quizzes on wiki pages and receive responses via email.
  • Check out ideas for classroom use at the Jotform blog.
  • Do you use Dropbox? If so, you can create a JotForm that allows easy uploads to your Dropbox.

 

Notes and Caveats:

 

  • JotForm does limit the number of surveys and responses you can administer each month without a paid plan. Keep in mind that you can always use Google Forms (via your GMail account) to create simple embed forms for collecting data or even student responses to assessments. Another option that's free for limited use is Wufoo.
